Multiple Individuals For accounts opened in the names of Multiple Individuals.
Sole Proprietorship For accounts opened in the name of a Sole Proprietorship not operated as a corporation, limited liability company, partnership, or trust.
Corporation For accounts opened in the name of a Corporation.
Limited Liability Company For accounts opened in the name of a Limited Liability Company.
General Partnership For accounts opened in the name of a General Partnership.
Limited Partnership For accounts opened in the name of a Limited Partnership.
Trust For accounts opened in the name of a Trust.

Commodity Trading Advisor ("CTA") If the account will be traded by someone other than the account owner, and the trader is registered as a Commodity Trading Advisor ("CTA").
Discretionary Account If the account will be traded by someone other than the account owner, and the trader is NOT registered as a Commodity Trading Advisor ("CTA").
Non-US Customer If the account is owned by a non - US customer.
Transfer If the account is currently with another brokerage company and the owner wishes to transfer the account to FCStone, LLC.
Additional Risk Disclosure If an account meets certain criteria, they may be asked to sign this form.
Authorization of Delivery of Statements by Electronic Media Email and fax consent form in PDF format.
Sole Proprietorship (d/b/a) Acknowledgement If an account wishes to add a d/b/a name to the account. Please note that this does not apply if the d/b/a name is operated as a Corporation, Partnership, Limited Liability Company, Trust or other form of legal entity.
